Please xerox the front and back of your insurance card, write your name and return phone number, social security number and date of birth and fax it to 703-689-2227, write that you'd like Blanca to please verify your insurance benefits for you.
We are providers for most insurance carrier. If you have "PPO", chances are you have Chiropractic coverage of some kind. We look forward to serving you.
